The patient is seen for followup
of his malignant lymphoma. The patient received chemotherapy
with CHOP plus Rituxan, with his last treatment being given
__________. He has been seen at routine intervals and has
felt no evidence of recurrent lymphoma. Presently, he is doing
quite well and has no specific complaint.
PAST MEDICAL HISTORY:
His past medical history includes hyperlipidemia and hypertension,
which is being followed by __________.
SOCIAL HISTORY:
The patient is retired.
FAMILY HISTORY:
Unchanged.
MEDICATIONS:
Include Zestril, Lipitor, vitamins, and a half tab of aspirin
daily.
REVIEW OF SYSTEMS:
Review of systems is negative for night sweats, weight loss,
and fever. No evidence of recurrent lymphadenopathy. The patient
had a recent CAT scan and has received copies of the results.
PHYSICAL EXAMINATION:
GENERAL: On
examination, patient presents as a well-developed, well-nourished,
middle-aged, white male in no
acute distress.
VITAL SIGNS:
Show weight 173 pounds. Temperature 97.1. Pulse is 62 and
regular. Respirations 16 per minute.
Blood pressure is 130/82.
HEENT: Head
is normocephalic. Eyes: Pupils equal. There is no scleral
icterus. Oropharynx is clear.
NECK: Supple
without lymphadenopathy or thyroid nodules.
CHEST: Clear
to auscultation.
HEART TONES:
Regular sinus rhythm.
LYMPH NODES:
There is no axillary or inguinal adenopathy.
ABDOMEN: Soft
without masses or organomegaly.
EXTREMITIES:
Unremarkable.
LABORATORY DATA:
CAT scan of the chest shows a focal pearl scar in the mid-left
lung but this small opacity shows no significant change from
his previous CAT scan. There are no hilar or mediastinal masses.
CAT scan of the abdomen shows normal liver, spleen, and pancreas.
There is no retroperitoneal mass or adenopathy. No change
is noted. CT of the pelvis shows no significant abnormality.
He does have multiple diverticula but there is no evidence
of diverticulitis.
